Before you submit your application

    Certificate of Correction (CCR) requests must now be filed online (Certificate of Correction form required). Certificate of Correction (CCR) requests will no longer be accepted via email.

    To submit a CCR, log in to FDNY Business, select “Initiate Application/Request” and then select “FDNY Summons Certificate of Correction Request” under “Enforcements.”

    Please note: that even if compliance is shown at the hearing, you are still required to file a Certificate of Correction. It is also important to understand that if you do not respond to an FDNY Summons, a default judgment imposing maximum penalties may be entered against you.

    Criminal Court Summonses: If you receive a Criminal Court Summons, you must appear at New York City Criminal Court. The Criminal Court Summons will indicate the date, time and location for the hearing. For assistance, contact FDNY.BusinessSupport@fdny.nyc.gov.

    Violation Orders and Vacate Orders: If you receive a Violation Order you must submit your request for reinspection on FDNY Business and attach a copy of the Violation Order to the request. If you have been issued a Vacate Order, please contact and send all proof of correction to the FDNY Customer Service Desk at FDNY.BusinessSupport@fdny.nyc.gov.

    Submit the completed application online with all supporting documents.

    You must make payment on FDNY Business to submit your application. To make payments for additional invoiced fees after submission use one of the following methods:
