      All outstanding fines imposed for Health Code Violations must be paid before you can apply for or renew a permit issued by the NYC Department of Health and Mental Hygiene. Fines may be paid in person at the NYC Department of Consumer and Worker Protection (DCWP) Citywide Licensing Center or Online.

        Potable well water for drinking purposes applicants must establish that the municipal water supply is not accessible. Applicants must also demonstrate that the water quality of the proposed well will meet the standards as outlined in Subpart 5-1 of the New York State Sanitary Code. This can be verified by the New York City Department of Environmental Protection sampling.

          Non-Potable well water for other than drinking purposes applicants must receive a Letter of Approval for Groundwater Discharge to Sanitary or Combined Sewer from the Department of Environment Protection. Please use the appropriate form available in the Links section above.

            New proposed well applicants must submit an assessment of the proposed site created by a licensed architect or engineer. The assessment should include:  the past land use of the area, and any potential sources of groundwater contamination that may migrate into the well point's cone of depression such as gas stations, chemical plants, landfills, transfer stations, etc.
            • 6
              New proposed well applicants must also submit a map or plan of the property, showing the location of any buildings, proposed well site, property boundaries, and any potential sources of pollution (fuel or septic tanks, solid waste, salt storage, etc.) within 100 feet of the well. A licensed architect or engineer must draft this map.

                Steps to apply online:

                Complete the questionnaire.

                  Gather all supporting documents required to apply for this permit.
                  • 3
                    Make individual electronic versions of each supporting document for uploading to the system. The following document types can be uploaded: PDF, JPG, GIF, PNG, XLS, XLSX, DOC, DOCX, VSD, and TXT.

                      Create an account with the NYC Online Licensing system. Payment is required to complete and submit the application so applicants should have their credit or debit card information readily available. Partially completed applications may be saved and returned to later for completion.

                        Select the type of permit for which you are applying and enter the requested information into each field.  Fields with an asterisk (*) are required.  Applicants will be requested to upload copies of required documents. Please upload each supporting document individually.

                          Pay with a credit or debit card to complete the application.
